Care Management
The Council on Aging offers care management services to help frail older and disabled adults remain living safely and independently in their own homes for as long as possible.
care managers take a holistic approach in addressing our clients’ needs. The care manager starts by completing a comprehensive in-home assessment of the person’s physical and mental abilities, emotional status, social support and living situation. Based on the assessment, the care manager develops an individual care plan with the client and /or family.
The care plan is reviewed by our clinical staff to ensure quality and completeness of the plan. Goals are set up to meet our client’s daily care needs and the foundation of the plan is to link each client with community services that will meet individual needs. In addition, respite is often provided to family caregivers. From installing grab bars in a bathtub to arranging for regular transportation to doctor’s appointments, our care management services can make the connections that can make all the difference in a person’s life.
