Linkages Facts
SERVICE GOALS
As the population ages, more and more older adults need care management' services. Our goal is to become more efficient in serving this population. Some of the issues we address are:
- Outreach to underserved ethnic and low-income groups
- Funding resources to provide services
- Affordable and available housing for older and disabled adults
- Transportation
- Home-delivered meals for disabled individuals under 60 years of age
- Affordable in-home care
- Low-cost prescription medications
PERFORMANCE TO DATE
In FY 2003-2004 care management program served 350 clients. Of these, a little over 60% of the clients were female and about 40% of them were male.

Our oldest client was 102 years old and the youngest was 26 at the time of service. Of the clients served, 19.2% were under the age of 60 and 80.8% were 60 years old or more.

64.6% of the clients were Caucasian, 26% were Hispanic, 8% were Asian/Pacific Islander and 1.4% were African American.
