- Is there a fee for Care Management services?
No. All Care Management Care Management services are free of charge, although donations are gladly accepted. In some instances, Care Management may also provide payment for selected services, such as installation of grab bars or respite care. In general, clients pay for most direct services.
- What does the assessment include?
A Care Management care manager will visit our client where he or she is currently living to take the individual’s history and to assess the extent to which the person needs assistance. Based on that assessment, the care manager will evaluate the individual’s ability to manage his or her affairs and activities of daily living.
- How are services delivered?
Based on the individual's needs, Care Management care managers make referrals to existing services in our local area.
- Does Care Management provide in-home help?
No. Care Management does not provide in-home support. The care manager will give several referrals to home care resources in the community.
- Does Care Management provide home delivered meals?
No. Care Management does not have a meal program. The care manager will refer the client to the appropriate meal program in their area.
- How does one receive help from Care Management?
Contact Care Management at (714) 479-0107. Eligibility will be determined and the initial intake will be done over the phone. The case will then be assigned and the care manager will schedule the initial in-home assessment.
- Are there Care Management programs in other areas of California?
Yes. There are Care Management Programs throughout the state. Call the Eldercare Locator Line at (800) 677-1116 to locate other programs in a specific area.
- Does Care Management have information on long-term care living options?
Yes. Even though our goal is to help individuals remain safe and independent at home, if the client wishes to have information about available facilities, Care Management can provide resources for other living options.
- How long does an individual remain part of the Care Management program?
Care Management remains in effect until the care plan has been implemented and services are in place. Care Management is not a long-term program. Usually Care Management serve clients from six months to one year.
