Adult Care Information System (ACIS)

The Adult Care Information System (ACIS) is the Council’s proprietary database that generates reports based on the public records known as 809 Reports which are generated by the State Department of Social Services, Community Care Licensing (CCL). 809 Reports document the results of visits made by CCL to residential care facilities for the elderly. CCL is a state regulatory agency responsible for license and enforcement activities. Each residential care facility has copies of their own 809 Reports which are available for public review.
The ACIS report identifies the following information for each Facility:

  • Name of Facility, listed alphabetically by city
  • Annual evaluation conducted by CCL and all complaint investigations
  • Complaints reported from various sources to CCL
  • Investigations conducted by CCL that may or may not result in deficiency citations
  • Deficiencies that generate a per day fine of $50.00 or more
  • Serious deficiencies recorded as Level A and Level B

The Council has made every effort to validate the completeness and accuracy of the quantitative information contained in the ACIS report. Obviously all complaints or deficiencies are not of equal seriousness. The ACIS report should be used as a screening tool and the user is cautioned to refer to the actual 809 Reports when detailed information concerning complaints or deficiencies if required.

Origins
 

The Ombudsman program began in 1972 and was established in all states under the Older American’s Act.
